One thing that normally sets a fantasy (or just medieval) setting are the professions in the hamlet, village or town. I use the word profession lightly, it is really a list of livelihoods. With a good list of realistic (and not so) occupants the players can and will use them to their advantage, but the DM can also flesh out their settlements with a plethora of individuals to burst the player's imaginations.
I know many of the names are sexist, but I did not invent them and 'Ferryperson' sounds silly!
